RESUMO

Alzheimer's disease (AD) is a neurodegenerative disease associated with aging and age-related cognitive decline. It is characterized by insidious onset and progressive development, and has become a major global health and socioeconomic issue. The exact mechanisms underlying AD have not been fully elucidated, and various hypotheses have been proposed by researchers based on different etiologies, including the amyloid β (Aβ) cascade hypothesis, Tau protein hyperphosphorylation hypothesis, mitochondrial dysfunction and oxidative stress hypothesis, and neurotransmitter hypothesis. Therefore, there is an urgent need for comprehensive interventions targeting multiple pathways, links, and targets. Based on traditional Chinese medicine (TCM) theory and modern research findings, kidney-tonifying and anti-aging Chinese medicines have unique advantages of toxicity reduction, long-lasting effects, and treating both the root cause and the symptoms. They have been shown to counteract immune-inflammatory responses, clear reactive oxygen species, exhibit antioxidant properties, inhibit abnormal aggregation of Aβ and Tau proteins, reduce neuronal apoptosis, regulate central neurotransmitters, and modulate gut microbiota in AD. In recent years, stem cell therapy has been explored for the treatment of AD through two strategies: endogenous activation and exogenous transplantation, thereby replenishing and replacing damaged neurons. However, factors such as blood-brain barrier permeability, targeted delivery to the affected area, immune rejection, and cell survival rate can affect the efficacy of stem cell transplantation. Therefore, combining stem cell therapy with medication and other methods can further enhance the effectiveness of stem cell transplantation. Kidney-tonifying and anti-aging Chinese medicines can activate dormant neural stem cells(NSCs) in the body, promote neuroregeneration, and facilitate tissue and organ repair and reconstruction in AD. The combined treatment of these Chinese medicines and stem cell transplantation has shown more significant efficacy compared to either treatment alone. This combination therapy provides a new integration point for the modernization of TCM and offers new ideas and approaches for the prevention and treatment of AD, as well as improving the effectiveness of stem cell transplantation.

RESUMO

Stroke is one of the major diseases threatening human health, with ischemic stroke accounting for about 70%. Ischemic stroke is characterized by complex pathological mechanism and high incidence, mobility and mortality. At present, the effective clinical treatment measures for ischemic stroke are limited, and it is urgent to develop new and effective treatment measures to improve the prognosis of patients. In recent years, bone marrow mesenchymal stem cells (BMSCs) transplantation has shown great therapeutic potential for a variety of diseases, including ischemic stroke, and has become a new research hotspot. However, due to the low homing and survival rate of BMSCs in human body after transplantation, their clinical effect on ischemic stroke needs to be further improved. With the characteristics of multi-components, multi-channels and multi-targets, Chinese medicine displays desirable curative effect on ischemic stroke, which has been widely concerned. Both Chinese medicine and BMSCs transplantation have good overall brain protection, and their combined effect on ischemic stroke is significantly better than that of single application. The mechanisms include improving the transplantation efficiency of BMSCs, promoting angiogenesis, enhancing neuroplasticity, ameliorating neuroinflammation, enhancing neuroprotection, and regulating the blood-brain barrier and exosomes. The combination of Chinese medicine and modern cutting-edge cell therapy reflects the advantages of integrative medicine, providing a new model and idea for preventing and treating ischemic stroke and improving the efficacy of BMSCs transplantation.

RESUMO

The cyclin-dependent protein kinase 9 (CDK9) is a member of the family of cyclin-dependent protein kinases.Different from other CDKs,CDK9 mainly works on the transcription regulation and has no affects on the cell cycle progress.There are many kinds of CDK9 inhibitors in the clinical research.The detailed structure and action mechanism of CDK9,its difference of protein structure from other CDKs,several selective or nonselective CDK9 inhibitors,as well as their structure-activity relationship (SAR) are discussed in this paper.

BACKGROUND:In the total hip arthroplasty, intravenous and topical tranexamic acid can significantly reduce the perioperative blood loss and blood transfusion rate, but at present, the clinical application of tranexamic acid is stil controversial. OBJECTIVE:Cochrane system evaluation method is applied on the efficacy and safety of intravenous combined with topical application versus single-dose intravenous application of tranexamic acid in the total hip arthroplasty. METHODS:We searched the Cochrane Library, PubMed, Ovid, EMBASE, CBM, Wanfang, VIP and CNKI database, from inception to July 2016 and retrieval of randomized control ed trials concerning intravenous combined with topical application and single dose intravenous application of tranexamic acid during total hip arthroplasty. Total blood loss, intraoperative blood loss, postoperative blood loss, blood transfusion rate, thrombosis, operation time, and length of hospital stay were analyzed by meta-analysis. RESULTS AND CONCLUSION:(1) There were seven randomized control ed trials with 620 patients. (2) Meta-analysis results indicated that total blood loss, intraoperative blood loss, postoperative blood loss, and blood transfusion rate were significantly better in the combination group than in the single dose group (P0.05). (3) These results suggested that compared with the single dose of intravenous application, intravenous combined with topical application, can significantly reduce the loss of blood volume and blood transfusion rate and did not increase the rate of thrombosis. BACKGROUND:Plate fixation or intramedulary fixation for clavicle fractures has remained a matter of debate. At present, there is a need for a higher level of evidence to evaluate the advantages and disadvantages of two different surgical methods. OBJECTIVE:To assess the clinical efficacy of intramedulary fixationversusplate fixation in the treatment of clavicle fractures. METHODS:We conducted a computerized search of the electronic databases like Cochrane Library, PubMed, Embase, OVID, CBM, CNKI, Wanfang and VIP for relevant randomized controled trials on plate fixation or intramedulary fixation for clavicle fractures. The quality of the included studies was independently assessed with RevMan 5.3 software. The meta-analysis was performed in Stata 13.0. RESULTS AND CONCLUSION:Eight randomized controlled trials with 527 patients were included in this meta-analysis. The meta-analysis results showed union time, surgery time, incision length, hospital stay and infection rate were less in the intramedullary fixation group than in the plate fixation group (P< 0.05). No significant difference in shoulder function score, upper limb function score, fracture nonunion and refracture was determined between the two groups (P< 0.05). The current evidences suggested that intramedullary fixation is the optimum choice in the treatment of clavicle fractures.

<p><b>OBJECTIVE</b>Due to random sampling of non-adaptive, high-quality reconstruction of the original signal, one-dimensional non-stationary multi-channel EEG signal can be achieved automatic detection and analysis.</p><p><b>METHODS</b>A new multicomponent redundant dictionaries with the atoms of the Gaussian function and its first and second derivatives was built in the paper, and reconstructed signal base on compressed sensing measurement model.</p><p><b>RESULTS</b>The selected dictionary atoms can more effectively match the EEG signals in a variety of transient characteristics of the waveform, allowing the formation of EEG signal is more sparse matching pursuit decomposition. With the theory based on compressed sensing signal sampling, only half of the original signal with different sample size can be used to reconstruct the original signal quality, the important instantaneous features of the waveform can well be maintained.</p><p><b>CONCLUSION</b>Signal sampling based on the theory of compressed sensing contains enough information of the original signal, using the prior conditions of EEG signals (or compressibility) sparsity, high-dimensional signal and original image can be reconstructed through a certain decoding of linear or nonlinear model.</p>

RESUMO

Objective To select algorithm for noninvasive EEG screening of epilepsy patients with a view to early detection and reduction of the incidence of epilepsy,morbidity and mortality.Methods Electroencephalogram(EEG)signal characteristics of the normal and epilepsy wave were extracted,automatically identified and classified based on support vector machine(SVM) analysis with a view to achieving epilepsy automatic scale screening.Results The EEG characteristics energy displayed by the model between epilepsy patients and healthy people could be divided obviously.As a new machine learning methods,SVM had a strong ability to generalize.EEG signals based on the algorithm of the classification would become diagnosis of epilepsy patients misprision of a new viable avenue.Conclusion SVM is suitable for the limited samples(small samples).The spontaneous EEG classification with SVM can achieve better results,so it can be used to epileptic EEG abnormality screening.
